I. – For operations with optional membership, the information sheet provided by the policyholder to the member at the time of enrolment includes, in addition to the information mentioned in the second paragraph of Article L. 141-4 :
a) The company name and contact details of the policyholder ;
b) The essential terms of the agreement, in particular the possibility of reducing the service value of the annuity unit and converting the agreement, which may be provided for in accordance with II of article L. 441-2 ;
c) A statement that the rights and obligations of the member may be modified by amendments to the said agreement and the procedures for the adoption of these amendments by the policyholder;
d) The conditions for exercising the waiver option and a drafting model designed to facilitate the exercise of this option;
e) The terms and conditions for converting the agreement into life annuities in accordance with the conditions laid down by order of the Minister for the Economy.
A summary of the key features of the agreement is included at the beginning of this notice, in a format specified by order of the same minister, which also sets out the content in a restrictive manner. This order also specifies all the information that must appear in the notice, in particular the essential stipulations within the meaning of b.
II. – For operations with optional membership, failure to provide the notice provided for in I of this article automatically results in an extension of the waiver period provided for in article L. 132-5-1, under the conditions provided for in paragraph 6 of article L. 132-5-2.
