The digital health space is made up of the following elements:
1° The holder’s administrative data: surname, first names, gender, date and place of birth, national health identifier mentioned in article L. 1111-8-1, postal, electronic and telephone contact details, where applicable, identity and contact details of his/her legal representatives or the person responsible for a representation measure relating to a person of age, contact details of the attending doctor;
2° His/her shared medical file, including the elements mentioned in article R. 1111-42;
3° a) Their health constants produced in particular by digital services or tools referenced in the catalogue mentioned in 6°;
b) A health questionnaire freely completed by the holder containing their current treatments, the most recent procedures they have undergone and their medical history;
c) Any other health data useful for the prevention, coordination, quality and continuity of care not included in the shared medical file in application of article R. 1111-42, filled in, with the consent of the holder, by a professional, a healthcare establishment, a social or medico-social establishment or service, or by means of a digital service or tool referenced in the catalogue mentioned in 6°;
4° Data relating to the reimbursement of his/her healthcare expenses by compulsory health insurance schemes;
5° a) A secure health messaging system enabling the holder to exchange messages and documents with professionals, health establishments and social or medico-social establishments or services, under conditions designed to ensure the security of the information thus transmitted;
b) A diary enabling the holder to organise events relating to his/her health, which may be supplied by the holder him/herself, by a professional, a health establishment, a social or medico-social establishment or service or by a digital service or tool referenced in the catalogue mentioned in 6°;
6° A catalogue of digital health tools and services referenced under the conditions set out in articles R. 1111-37, R. 1111-38 and R. 1111-39, offering, in particular, telehealth services, services developed to promote prevention and make care pathways more fluid, homecare services, services providing assistance with guidance and assessing the quality of care, services aimed at informing users about the care on offer and the rights to which they are entitled;
7° Where applicable, data relating to the reception and support provided by the social and medico-social establishments and services mentioned in article L. 312-1 of the Social Action and Family Code, which may be entered by the holder him/herself, by a professional, a health establishment, a social or medico-social establishment or service, or by means of a digital service or tool referenced in the catalogue mentioned in 6° of this article. This data relates in particular to the social and medico-social assessment of individuals with a view to providing appropriate support, the development and monitoring of the individual’s reception and support project, and coordination between social, medical and paramedical professionals;
8° A directory of authorisations for access to all or part of the holder’s digital health space and, where applicable, to health data concerning the holder processed outside this space, given by the holder to professionals, establishments and services or digital health tools.
