A practitioner who refers a patient to a health care establishment must include with his request a liaison letter summarising the information he has at his disposal at the place where he is treating the patient, in accordance with the conditions laid down in article L. 1110-4. This letter includes, in particular, the reasons for the request for hospitalisation, current treatments and known allergies. It is sent by secure messaging system that meets the conditions laid down in article L. 1110-4-1, or by any other means that guarantees the confidentiality of the information. It is placed in the patient’s shared medical record if this record has been created, or is given to the patient.
When article L. 3213-1 is applied, the detailed medical certificate takes the place of a liaison letter for admission to the host establishment.
