Article R1112-1-1 of the French Public Health Code

A practitioner who refers a patient to a health care establishment must include with his request a liaison letter summarising the information he has at his disposal at the place where he is treating the patient, in accordance with the conditions laid down in article L. 1110-4. This letter includes, in particular, the reasons for the request for hospitalisation, current treatments and known allergies. It is sent by secure messaging system that meets the conditions laid down in article L. 1110-4-1, or by any other means that guarantees the confidentiality of the information. It is placed in the patient’s shared medical record if this record has been created, or is given to the patient.

When article L. 3213-1 is applied, the detailed medical certificate takes the place of a liaison letter for admission to the host establishment.

Le praticien qui adresse un patient à un établissement de santé accompagne sa demande d’une lettre de liaison synthétisant les informations nécessaires à la prise en charge du patient dont il dispose sur son lieu d’intervention, dans le respect des conditions prévues à l’article L. 1110-4. Cette lettre comprend notamment les motifs de la demande d’hospitalisation, les traitements en cours et les allergies connus. Elle est adressée par messagerie sécurisée répondant aux conditions prévues à l’article L. 1110-4-1, ou par tout moyen garantissant la confidentialité des informations. Elle est versée dans le dossier médical partagé du patient si ce dossier a été créé, ou lui est remise.

Lorsqu’il est fait application de l’article L. 3213-1, le certificat médical circonstancié tient lieu de lettre de liaison pour l’admission dans l’établissement d’accueil.
