I.-When the patient is discharged from the healthcare establishment, a liaison letter, drawn up by the doctor in the establishment who treated the patient, is given to the patient by this doctor, or by another member of the healthcare team mentioned in 1° of article L. 1110-12 who treated the patient, and who ensures that the information required for continuity of care has been understood.
In compliance with the provisions of articles L. 1110-4 and L. 1111-2, the liaison letter is sent on the same day to the attending doctor and, where applicable, to the practitioner who referred the patient. It is sent by secure messaging system that meets the conditions laid down in article L. 1110-4-1, or by any means that guarantees the confidentiality of the information, and placed in the patient’s shared medical record if this record has been created.
II-This liaison letter contains the following information:
1° Identification of the patient, the attending doctor and, where applicable, the referring practitioner, as well as identification of the doctor in the health establishment who treated the patient, with the dates and procedures for admission to and discharge from hospital ;
2° Reason for hospitalisation ;
3° Medical summary of the stay, specifying any undesirable events that occurred during hospitalisation, the identification of multi-resistant or emerging micro-organisms, the administration of blood products or blood derivatives, or the fitting of an implantable medical device;
4° Treatments prescribed on discharge from the establishment (or discharge prescriptions) and those discontinued during the stay and the reason for discontinuation or replacement, specifying, in particular for medicinal treatments, the dosage and duration of treatment;
5° Announcement, where appropriate, that you are awaiting test results or other information to supplement this liaison letter;
6° Follow-up, where appropriate, including medical and social follow-up, such as planned and scheduled procedures, recommendations and special monitoring.
