The request to consult the register is made by the director of the health establishment in which the removal is planned or, failing this, by another official of the establishment expressly authorised for this purpose by the director.
The request must be accompanied by a copy of the death certificate provided for in article R. 1232-3.
La demande d’interrogation du registre est formée par le directeur de l’établissement de santé dans lequel le prélèvement est envisagé ou, à défaut, par un autre responsable de l’établissement expressément habilité à cet effet par le directeur.
La demande est accompagnée de la copie du procès-verbal du constat de la mort prévu par l’article R. 1232-3.
