The death certificate referred to in article L. 1232-1 is drawn up on a document, the model for which is set by order of the Minister for Health.
When the death certificate is drawn up for a person in persistent cardiac and respiratory arrest, the report shall indicate the results of the clinical findings and the date and time of the death certificate. The report must be drawn up and signed by a doctor who meets the conditions set out in article L. 1232-4 .
When death is declared in the case of a person assisted by mechanical ventilation and retaining haemodynamic function, the death certificate shall indicate the results of the concordant clinical findings of two doctors who meet the conditions set out in article L. 1232-4. It also mentions the results of the examinations defined in 1° or 2° of article R. 1232-2, as well as the date and time of this report. This report is signed by the two doctors mentioned above.
The death certificate is signed at the same time as the death certificate provided for by order of the Minister for Health.
