I. – The person selected at random is required to provide the following information on the information form:
– marital status, address and family situation;
– personal or professional telephone number and e-mail address;
– professional situation;
– holding public elected office or judicial office;
– having served as a juror or citizen assessor during the five years preceding the current year;
– activities, including associative activities, connected with the judicial institution;
– date and nature of offences of which the person concerned, their spouse, their partner in a civil solidarity pact, their cohabitee or one of their parents or direct relatives may have been a victim in the current year or in the two previous years;
– guardianship, curatorship or safeguard of justice measure to which they are subject.
II. – The person drawn by lot may submit a request for exemption from jury or citizen assessor duties in the collection of information, giving reasons in accordance with article 258.
III – It may supplement this collection with any other information it deems useful. They are informed that they may, in particular, specify the periods during which, in the event of being appointed as a citizen assessor, they would like, as far as possible, not to be called upon to sit as well as the reasons for their unavailability.
