Any person applying for approval to run a children’s home for the disabled must provide in support of their application :
1° A police record less than three months old ;
2° A copy of their diplomas and a list of their qualifications, with supporting documents;
3° A curriculum vitae indicating, in particular, where they have lived for the last ten years and the professions they have practised;
4° A list of relatives who will be staying at the establishment;
5° A medical certificate issued by a doctor of the applicant’s choice stating :
a) That the person concerned is physically fit and in good health to carry out the duties of director of a children’s home for the elderly ;
b) That he has fulfilled the obligations laid down by the legislation on vaccinations.
The application and accompanying documents must be sent to the Director General of the Regional Health Agency.
