Article R2393-25 of the French Public procurement code

Where the declaration of subcontracting is made when the tender is submitted, the tenderer shall provide the purchaser with a declaration containing all of the following information:
1° The nature of the subcontracted services;
2° The name, business name or corporate name and address of the proposed subcontractor;
3° The place of performance of the subcontracted services;
4° The maximum amount of the sums to be paid to the subcontractor;
5° The terms of payment provided for in the proposed subcontract and, where applicable, the price variation terms;
6° Where applicable, the subcontractor’s capacities on which the tenderer is relying.
The tenderer shall also provide the purchaser with a declaration from the subcontractor stating that he is not in any of the cases of exclusion mentioned in the legislative provisions of Chapter I of Title IV.

Lorsque la déclaration de sous-traitance intervient au moment du dépôt de l’offre, le soumissionnaire fournit à l’acheteur une déclaration mentionnant l’ensemble des informations suivantes :
1° La nature des prestations sous-traitées ;
2° Le nom, la raison ou la dénomination sociale et l’adresse du sous-traitant proposé ;
3° Le lieu d’exécution des prestations sous-traitées ;
4° Le montant maximum des sommes à verser au sous-traitant ;
5° Les conditions de paiement prévues par le projet de sous-traité et, le cas échéant, les modalités de variation des prix ;
6° Le cas échéant, les capacités du sous-traitant sur lesquelles le candidat s’appuie.
Le soumissionnaire remet également à l’acheteur une déclaration du sous-traitant indiquant qu’il n’est pas placé dans un cas d’exclusion mentionnés par les dispositions législatives du chapitre Ier du titre IV.
