I. – The priority of the territorial mental health project is to organise a high-quality and seamless health and life pathway, particularly for people suffering from serious and long-term mental disorders, who are or are at risk of mental disability, with a view to their recovery and social inclusion.
To this end, it provides for :
1° Actions designed to prevent the onset or worsening of the disability, through the earliest possible access to care, particularly rehabilitation care, and to social and medico-social support;
2° The development of adapted and diversified services designed to facilitate people’s access to housing, employment, schooling, studies and social life, aiming as far as possible to integrate and remain in an ordinary environment.
II. – To enable these objectives to be achieved, the territorial mental health project is based on the implementation of coordinated monitoring by local health, social and medico-social players.
