Article R3224-6 of the French Public Health Code

I. – The priority of the territorial mental health project is to organise a high-quality and seamless health and life pathway, particularly for people suffering from serious and long-term mental disorders, who are or are at risk of mental disability, with a view to their recovery and social inclusion.

To this end, it provides for :

1° Actions designed to prevent the onset or worsening of the disability, through the earliest possible access to care, particularly rehabilitation care, and to social and medico-social support;

2° The development of adapted and diversified services designed to facilitate people’s access to housing, employment, schooling, studies and social life, aiming as far as possible to integrate and remain in an ordinary environment.

II. – To enable these objectives to be achieved, the territorial mental health project is based on the implementation of coordinated monitoring by local health, social and medico-social players.

I. – Le projet territorial de santé mentale a pour priorité l’organisation du parcours de santé et de vie de qualité et sans rupture, notamment pour les personnes souffrant de troubles psychiques graves et s’inscrivant dans la durée, en situation ou à risque de handicap psychique, en vue de leur rétablissement et de leur inclusion sociale.

A ce titre, il prévoit :

1° Les actions destinées à prévenir la survenue ou l’aggravation du handicap, par l’accès le plus précoce possible aux soins notamment de réhabilitation, et aux accompagnements sociaux et médico-sociaux ;

2° Le développement de services adaptés et diversifiés destinés à faciliter l’accès des personnes au logement, à l’emploi, à la scolarisation, aux études et à la vie sociale, en visant le plus possible l’insertion et le maintien en milieu ordinaire.

II. – Pour permettre l’atteinte de ces objectifs, le projet territorial de santé mentale s’appuie sur la mise en place d’un suivi coordonné réalisé par des acteurs sanitaires, sociaux et médico-sociaux de proximité.
