I.-The application by which an online public communication service requests removal from the list referred to in I of Article L. 331-25 shall be sent to the Autorité de régulation de la communication audiovisuelle et numérique by registered letter with acknowledgement of receipt.
The application referred to in the previous paragraph shall include:
1° In the case of a natural person, their surname, first name, address, telephone number and e-mail address and, if they are subject to formalities for registration in the Trade and Companies Register or the National Register of Companies as a company in the trades and crafts sector, their registration number;
In the case of a legal person, their name, first name, address, telephone number and e-mail address and, if they are subject to formalities for registration in the Trade and Companies Register or the National Register of Companies as a company in the trades and crafts sector, their registration number
2° In the case of a legal entity, its name or company name, the telephone and electronic contact details of the natural person to be contacted, and, in the case of a company subject to formalities for registration in the Trade and Companies Register or the National Register of Companies as a company in the trades and crafts sector, its registration number and the address of its registered office and any document attesting to these formalities;
4° An undertaking to respond to any requests for information required by the authority to verify compliance with copyright and related rights;
> The application must be written in French.

If the application is not accompanied by these elements, a request for regularisation is sent to the applicant or his authorised representative, who must reply and provide the additional information within one month.
