Article R3711-5 of the French Public Health Code

Any practitioner wishing to act as coordinating doctor must submit a request to the public prosecutor. This application must be accompanied by the following information and documents:

1° Nature of professional activities, places and dates of practice ;

2° Copies of qualifications and diplomas;

3° Proof of at least three years’ registration on the Roll of the Ordre des Médecins and the absence of any disciplinary sanctions as referred to in article R. 3711-3, as well as any suspension under article L. 4122-3;

4° Where applicable, a training certificate.

Le praticien qui souhaite exercer les fonctions de médecin coordonnateur adresse une demande au procureur de la République. Cette demande est assortie des renseignements et documents suivants :

1° Nature des activités professionnelles, lieux et dates d’exercice ;

2° Copies des titres et diplômes ;

3° Attestation justifiant d’au moins trois ans d’inscription au tableau de l’ordre des médecins et de l’absence de sanctions disciplinaires mentionnées à l’article R. 3711-3, ainsi que de suspension au titre de l’article L. 4122-3 ;

4° Le cas échéant, attestation de formation.
