Article R4002-6 of the French Public Health Code

An order of the Minister for Health sets out:

1° The composition of the file produced in support of the application for partial access;

2° The model form for the declaration of provision of services in the event of partial access and the list of supporting documents;

3° The content of the decision to authorise partial practice.

Un arrêté du ministre chargé de la santé fixe :

1° La composition du dossier produit à l’appui de la demande d’accès partiel ;

2° Le modèle de formulaire de la déclaration de prestation de services en cas d’accès partiel ainsi que la liste des pièces justificatives ;

3° Le contenu de la décision d’autorisation d’exercice partiel.
