Article R4041-8 of the French Public Health Code

The representative referred to in II of article R. 4041-7 shall send the application for registration to the council of the professional body with territorial jurisdiction by any means, including electronic means, that provide a date certain of receipt. The application must include:

1° A copy of the company’s articles of association and appendices;

2° A copy of the company’s Kbis extract.

Le mandataire mentionné au II de l’article R. 4041-7 adresse au conseil de l’ordre territorialement compétent la demande d’inscription par tout moyen, y compris dématérialisé, donnant date certaine à sa réception. La demande comprend :

1° Un exemplaire des statuts de la société et de ses annexes ;

2° Un exemplaire de l’extrait Kbis de la société.
