Article R4153-45 of the French Labour Code

The employer or the head of the establishment declaring the derogation shall make available to the Labour Inspectorate monitoring officer, from the time each young person is assigned to the work in question, information relating to:

1° The young person’s forenames, surname and date of birth ;

2° The vocational training followed, its duration and known training locations;

3° A medical opinion stating that the young person is fit to carry out the work in question;

4° the safety information and training provided to the young person under Articles L. 4141-1 to L. 4141-3;

5° The first names, surnames and position or function of the competent person or persons responsible for supervising the young person during the performance of the work in question.

L’employeur ou le chef d’établissement qui déclare déroger tient à disposition de l’agent de contrôle de l’inspection du travail, à compter de l’affectation de chaque jeune aux travaux en cause, les informations relatives :

1° Aux prénoms, nom et date de naissance du jeune ;

2° A la formation professionnelle suivie, à sa durée et aux lieux de formation connus ;

3° A l’avis médical d’aptitude à procéder à ces travaux ;

4° A l’information et la formation à la sécurité prévues aux articles L. 4141-1 à L. 4141-3, dispensées au jeune ;

5° Aux prénoms, nom, et qualité ou fonction de la personne ou des personnes compétentes chargées d’encadrer le jeune pendant l’exécution des travaux en cause.
