The employer or the head of the establishment declaring the derogation shall make available to the Labour Inspectorate monitoring officer, from the time each young person is assigned to the work in question, information relating to:
1° The young person’s forenames, surname and date of birth ;
2° The vocational training followed, its duration and known training locations;
3° A medical opinion stating that the young person is fit to carry out the work in question;
4° the safety information and training provided to the young person under Articles L. 4141-1 to L. 4141-3;
5° The first names, surnames and position or function of the competent person or persons responsible for supervising the young person during the performance of the work in question.
