The authorisation is issued for a period of five years and is renewable under the conditions set out in article R. 4211-9.
It mentions the surname, first name and position of the person preparing and supplying allergens, as well as the address of the place of preparation.
It specifies the pharmaceutical forms and routes of administration of the allergens.
It also includes the list of parent preparations as defined in 4° of article R. 4211-1.
It is accompanied by information sheets intended to be dispensed with allergens, as well as the model treatment booklet mentioned above.
