Article R4222-2 of the French Public Health Code

The pharmacist applying for registration on the Register shall provide the documents referred to in article R. 4112-1, with the exception of 3°.

He shall also provide a copy, accompanied if necessary by a translation made by a translator approved by the French courts or authorised to intervene before the judicial or administrative authorities of a Member State of the European Union or a party to the Agreement on the European Economic Area, of one of the training qualifications required by 1° of article L. 4221-1, to which are attached :

1° Where the applicant is a national of a Member State of the European Union or a party to the Agreement on the European Economic Area: the certificate(s) provided for by the texts adopted in application of articles L. 4221-4 and L. 4221-5 ;

2° If the applicant holds an authorisation to practise issued pursuant to articles L. 4221-9, L. 4221-12, L. 4221-14-1 and L. 4221-14-2 or an authorisation to practise issued pursuant to article L. 4002-3, a copy of this authorisation.

The president of the central council of the order or the president of the regional council of the order of dispensing pharmacists acknowledges receipt of the application within one month of its receipt and, if necessary, informs the applicant within the same time limit of any missing documents and information.

Article R4222-2

Le pharmacien qui demande son inscription au tableau fournit les pièces mentionnées à l’article R. 4112-1, à l’exception du 3°.

Il fournit en outre une copie, accompagnée le cas échéant d’une traduction faite par un traducteur agréé auprès des tribunaux français ou habilité à intervenir auprès des autorités judiciaires ou administratives d’un Etat membre de l’Union européenne ou partie à l’accord sur l’Espace économique européen, de l’un des titres de formation exigés par le 1° de l’article L. 4221-1, à laquelle sont joints :

1° Lorsque le demandeur est un ressortissant d’un Etat membre de l’Union européenne ou partie à l’accord sur l’Espace économique européen : la ou les attestations prévues par les textes pris en application des articles L. 4221-4 et L. 4221-5 ;

2° Lorsque le demandeur bénéficie d’une autorisation d’exercice délivrée en application des articles L. 4221-9, L. 4221-12, L. 4221-14-1 et L. 4221-14-2 ou d’une autorisation d’exercice délivrée en application de l’article L. 4002-3, la copie de cette autorisation.

Le président du conseil central de l’ordre ou le président du conseil régional de l’ordre des pharmaciens d’officine accuse réception de la demande dans un délai d’un mois à compter de sa réception et le cas échéant, indique au demandeur dans ce même délai les pièces et informations manquantes.
