As part of the medical prescription, the speech and language therapist draws up an assessment which includes the speech and language therapy diagnosis, the objectives and the treatment plan. The report on this assessment is sent to the prescribing doctor, together with any information in the speech and language therapist’s possession and any advice that may be useful to the doctor in establishing the medical diagnosis, in clarifying the technical aspect of the planned rehabilitation and in enabling him/her to adapt the treatment according to the person’s state of health and how it is developing.
