Article R4427-2 of the French Labour Code

The declaration of the first use of biological pathogens must include :
1° The name and registered office of the company and the address of the establishment;
2° The name and address of the occupational physician;
3° The name and position of the safety officer, if any, in the workplace;
4° The result of the assessment of the risks of exposure to biological agents;
5° The species or, failing that, the genus to which each biological agent concerned belongs;
6° The protection and prevention measures envisaged.

La déclaration d’une première utilisation d’agents biologiques pathogènes comprend :
1° La dénomination et le siège social de l’entreprise et l’adresse de l’établissement ;
2° Le nom et l’adresse du médecin du travail ;
3° Le nom et la qualité du responsable sécurité, s’il existe, sur le lieu de travail ;
4° Le résultat de l’évaluation des risques d’exposition à des agents biologiques ;
5° L’espèce ou, à défaut, le genre auquel appartient chaque agent biologique concerné ;
6° Les mesures de protection et de prévention envisagées.
