Article R4441-12 of the French Public Health Code

Each candidate completes a declaration of candidacy in which he or she indicates his or her full name, date of birth, address, qualifications, method of practice, professional qualification and, where applicable, his or her current and, where applicable, past membership of the professional order or professional bodies.

The list of candidates is initialled by the president of the body of the ordre or, in the case provided for in the second paragraph of I of article R. 4441-4, by the president of the Conseil national.

Chaque candidat remplit une déclaration de candidature dans laquelle il indique ses nom et prénoms, sa date de naissance, son adresse, ses titres, son mode d’exercice, sa qualification professionnelle et, le cas échéant, ses fonctions ordinales ou dans les organismes professionnels, actuelles et, le cas échéant, passées.

La liste des candidats est paraphée par le président de l’organe de l’ordre ou, dans le cas prévu au deuxième alinéa du I de l’article R. 4441-4, par le président du Conseil national.
