Article R4453-32 of the French Labour Code

The application for authorisation must include :

1° The name and registered office of the company and the address of the establishment;

2° The name and address of the occupational health service to which it reports;

3° The name and position of the electromagnetic field risk prevention advisor appointed by the employer;

4° The result of the assessment of the risks of exposure to electromagnetic fields;

5° The circumstances justifying this approach;

6° The measures and means of protection envisaged;

7° A list of the workstations concerned;

8° Where applicable, the specific measures taken in the event of work being carried out by an external company;

9° The opinion of the occupational physician and the opinion of the social and economic committee.

La demande d’autorisation comprend :

1° La dénomination et le siège social de l’entreprise et l’adresse de l’établissement ;

2° Le nom et l’adresse du service de santé au travail dont il relève ;

3° Le nom et la qualité du conseiller à la prévention des risques liés aux champs électromagnétiques désigné par l’employeur ;

4° Le résultat de l’évaluation des risques d’exposition aux champs électromagnétiques ;

5° Les circonstances qui justifient cette démarche ;

6° Les mesures et moyens de protection envisagés ;

7° La liste des postes de travail concernés ;

8° Le cas échéant, les dispositions particulières prises dans le cadre de travaux réalisés par une entreprise extérieure ;

9° L’avis du médecin du travail et l’avis du comité social et économique.
