Article R512-16-4 of the French Consumer Code

The order and delivery of the goods shall be recorded in a report containing, in addition to a description of the goods and an indication of the price paid when the order was placed, including transport costs, the following information:

1° The surname, first names, position and place of residence of the authorised agent;

2° The date and time of the order;

3° The date and time of delivery of the goods;

4° Identification of the person from whom the goods were ordered: in the case of a natural person, their surname and first names, and in the case of a legal entity, its company name and the address at which it is established, where applicable, the name of the online sales site, where this information is known;

5° Where applicable, the registration number in the Trade and Companies Register or in the National Register of Companies as a company in the trades and crafts sector;

6° The serial number of the direct debit;

>The signature of the agent
7° The signature of the authorised agent.

La commande et la livraison de la marchandise sont relatées dans un procès-verbal comportant, outre une description de la marchandise et l’indication du prix payé à la commande, y compris les frais de transport, les mentions suivantes :

1° Les nom, prénoms, qualité et résidence de l’agent habilité ;

2° La date et l’heure de la commande ;

3° La date et l’heure de la livraison de la marchandise ;

4° L’identification de la personne à laquelle la marchandise a été commandée : s’il s’agit d’une personne physique, ses nom et prénoms, et, s’il s’agit d’une personne morale, sa raison sociale et l’adresse où elle est établie, le cas échéant, le nom du site de vente en ligne, lorsque ces informations sont connues ;

5° Le cas échéant, le numéro d’inscription au registre du commerce et des sociétés ou au Registre national des entreprises en tant qu’entreprise du secteur des métiers et de l’artisanat ;

6° Le numéro d’ordre du prélèvement ;

7° La signature de l’agent habilité.
