Article R5124-74 of the French Public Health Code

The declaration provided for in Article L. 5124-20 shall be sent to the Agence nationale de sécurité du médicament et des produits de santé by the person carrying out the brokering activity or by the legal representatives of the company by any means enabling receipt to be acknowledged.

The declaration includes administrative information relating to the company, including the name of the legal representatives of the company or, if the company is owned by a company, its legal form, its corporate name and the address of its registered office.

The form and content of the document containing the list of administrative information shall be determined by decision of the Director General of the Agency.

The Director General of the Agency may request any additional information.

Any change to the administrative information shall be notified to the Agency without delay by any means capable of acknowledging receipt.

The cessation of activity of the establishment shall be notified without delay in the same way.

Article R5124-74

La déclaration prévue à l’article L. 5124-20 est adressée à l’Agence nationale de sécurité du médicament et des produits de santé par la personne exerçant l’activité de courtage ou par les représentants légaux de l’entreprise par tout moyen permettant d’en accuser réception.

La déclaration comprend les renseignements administratifs relatifs à l’entreprise comprenant le nom des représentants légaux de l’entreprise, de l’entreprise ou, dans le cas où l’entreprise est la propriété d’une société, sa forme juridique, sa dénomination sociale et l’adresse du siège social de l’entreprise.

La forme et le contenu du document comportant la liste des renseignements administratifs sont fixés par décision du directeur général de l’agence.

Le directeur général de l’agence peut requérir toute information complémentaire.

Toute modification des renseignements administratifs est notifiée sans délai à l’agence par tout moyen permettant d’en accuser réception.

La cessation d’activité de l’établissement est communiquée sans délai dans les mêmes formes.
