Article R6312-28 of the French Public Health Code

Payment for a medical transport service provided by an approved company gives rise to the issue of a supporting document, a duplicate of which is kept by the company for one year and which includes :

1° A detailed account of the services actually provided and the corresponding sums claimed, as well as their total amount;

2° The day and time of transport;

3° The name and address of the company and the number and date of the authorisation;

4° The registration number of the vehicle or aircraft;

5° The names of the members of the crew or the driver;

6° The name of the doctor or nurse mentioned in 3° of article R. 6312-24.

Article R6312-28

Le paiement d’une prestation de transport sanitaire fait par une entreprise agréée donne lieu à la délivrance d’une pièce justificative dont le double est conservé par l’entreprise pendant un an et qui comporte :

1° Le décompte détaillé des prestations effectivement fournies et des sommes correspondantes qui sont réclamées ainsi que leur montant total ;

2° Le jour et l’heure du transport ;

3° Le nom et l’adresse de l’entreprise et le numéro et la date de l’agrément ;

4° Le numéro d’immatriculation du véhicule ou de l’aéronef ;

5° Le nom des membres de l’équipage ou du conducteur ;

6° Le nom du médecin ou de l’infirmier ou l’infirmière mentionné au 3° de l’article R. 6312-24.
