Companies must either issue policies under a continuous numbering system that may comprise several series, without omission or duplication, with successive riders being attached to the original policy, or assign policyholders or members continuous numbers that meet the same requirements.
Information relating to these documents must be easily accessible at all times and must include at least the following information :
– either the number of the contract or rider, or the number of the policyholder or member-policyholder with all the contracts or riders concerning him or her;
– date of subscription, duration of the contract ;
– name of the policyholder or insured party;
– if applicable, name or code of intermediary;
– date and time of commencement stipulated in the contract;
– date and reason for any withdrawal;
– currency in which the contract is denominated;
– type of cover by reference to the categories of insurance defined in article A. 344-2 ;
– amount of the guarantee limits, capital or annuity insured.
