Article D231-1-4-1 of the French Sports Code

For minors, with a view to obtaining or renewing a licence or registering for a sporting competition as referred to in article L. 231-2-1, the athlete and the persons exercising parental authority must jointly complete a questionnaire relating to their state of health, the content of which is specified by joint order of the Minister for Health and the Minister for Sport.

The persons exercising parental authority over the minor athlete certify to the federation that each section of the questionnaire has been answered in the negative. If this is not the case, they are required to produce a medical certificate attesting to the absence of contraindication to the practice of the sport or discipline concerned, dated less than six months.

Pour les personnes mineures, en vue de l’obtention ou du renouvellement de la licence ou en vue de l’inscription à une compétition sportive visée à l’article L. 231-2-1, le sportif et les personnes exerçant l’autorité parentale renseignent conjointement un questionnaire relatif à son état de santé dont le contenu est précisé par arrêté conjoint du ministre chargé de la santé et du ministre chargé des sports.


Les personnes exerçant l’autorité parentale sur le sportif mineur attestent auprès de la fédération que chacune des rubriques du questionnaire donne lieu à une réponse négative. A défaut, elles sont tenues de produire un certificat médical attestant de l’absence de contre-indication à la pratique du sport ou de la discipline concernée datant de moins de six mois.
