Article D3512-9-5 of the French Public Health Code

I.-The application for approval, accompanied by the information and documents listed in Annex I to Decree No. 2016-1139 of 22 August 2016, shall be sent by the head of the laboratory to the public establishment mentioned in Article L. 3512-15.

This application, together with the information and documents requested, must be submitted no later than 30 June of the calendar year preceding the year for which approval is sought.

II-Any application for renewal or modification of approval, accompanied by the information and documents specified in Annex II or Annex III of the same decree, must be sent by the head of the laboratory to the public establishment referred to in I, no later than 180 days before the expiry date of the approval or before the planned date of implementation of the modifications requested.

III – The information and documents provided by the head of the laboratory for the purposes of applying for approval, its renewal or its amendment shall be written in French.

I.-La demande d’agrément, accompagnée des informations et pièces figurant à l’annexe I du décret n° 2016-1139 du 22 août 2016, est adressée par le responsable du laboratoire à l’établissement public mentionné à l’article L. 3512-15.

Cette demande, accompagnée des informations et pièces demandées, est déposée au plus tard le 30 juin de l’année civile précédant l’année à compter de laquelle l’agrément est sollicité.

II.-Toute demande de renouvellement ou de modification d’agrément, accompagnée des informations et pièces prévues à l’annexe II ou à l’annexe III du même décret, est adressée par le responsable du laboratoire à l’établissement public visé au I, au plus tard 180 jours avant la date d’expiration de l’agrément ou avant la date envisagée de la mise en application des modifications demandées.

III.-Les informations et pièces fournies par le responsable du laboratoire en vue de la demande d’agrément, de son renouvellement ou de sa modification sont rédigées en langue française.
