The care structures mentioned in article D. 6124-301 are required to organise continuity of care outside their opening hours, including Sundays and public holidays. To this end, they must equip themselves with a medical device for immediate referral of patients.
In the event that the facility is unable to ensure continuity of care itself, it is required to sign an agreement with another health care facility that admits patients with the same medical condition(s) as those treated by the facility on a full-time basis. This agreement defines in particular the conditions under which patients under the care of the facility, if necessary, are either transferred to the other establishment, or referred to it, once they have left the facility, in order to ensure continuity of care.
Without prejudice to the preceding provisions, when the care structure practices ambulatory anaesthesia or surgery, each patient receives a discharge form before leaving the structure. This report, signed by one of the facility’s doctors, mentions the identity of the medical staff who took part in the operation, the recommendations on what to do in terms of post-operative or anaesthetic monitoring, particularly with regard to pain management, and the contact details of the healthcare establishment’s staff ensuring continuity of care.
