Article D6323-5 of the French Public Health Code

For each patient cared for in a health centre, a file containing all the health information required for diagnostic and therapeutic decisions is compiled in compliance with the confidentiality and ethical rules specific to the health professionals concerned.

The file includes the identification of the patient as well as, where applicable, that of the trusted support person defined in article L. 1111-6 and that of the person to be notified.

The content of this file guarantees the traceability of actions carried out in the context of each patient’s care.

Article D6323-5

Pour chaque patient pris en charge dans un centre de santé, un dossier comportant l’ensemble des informations de santé nécessaires aux décisions diagnostiques et thérapeutiques est constitué dans le respect de la confidentialité et des règles déontologiques propres aux professionnels de santé concernés.

Le dossier comporte l’identification du patient ainsi que, le cas échéant, celle de la personne de confiance définie à l’article L. 1111-6 et celle de la personne à prévenir.

Le contenu de ce dossier garantit la traçabilité des actions effectuées dans le cadre de la prise en charge de chaque patient.
