In order to meet the needs identified as part of the territorial diagnostics referred to in III of article L. 1434-10 and on the basis of the health projects of primary care teams, the health projects of specialised care teams, the health projects of territorial professional health communities where these have been validated, and the territorial health projects referred to in the same article L. 1434-10, the regional health agency may conclude territorial health contracts.
The territorial health contract defines the action taken by its signatories, their missions and commitments, the resources they devote to it and the methods of financing, monitoring and evaluation. To this end, the Director General of the Regional Health Agency may allocate funds from the regional intervention fund referred to in article L. 1435-8.
The territorial health contract is published on the website of the Regional Health Agency to enable public and private health establishments, medico-social structures, the liberal professions and representatives of approved user associations to find out about the actions and financial resources of the project.
Primary care teams and members of territorial professional health communities may benefit from the functions of coordination support schemes and the specific regional schemes mentioned in articles L. 6327-2 and L. 6327-6.
When it concerns an element of the armed forces health service, the territorial health contract is subject to the agreement of the Minister for Defence and is equivalent to an amendment to the specific contract provided for in article L. 6147-12.
