When a birth is registered, a health record is issued free of charge for each child. This booklet is issued by the civil registrar; failing this, it can be requested from the departmental maternal and child protection service.
A ministerial order shall determine the model and method of use of this booklet, which must include the results of the medical examinations provided for in articles L. 2132-2 and L. 2132-2-1 and in which all important observations concerning the child’s health must be noted as they occur.
The booklet is made out in the child’s name. It is given to the parents or persons exercising parental authority or to the persons or services to whom the child has been entrusted. They must be informed that no one may demand to see it and that any person called upon, by virtue of their position, to take cognisance of the information contained therein is bound by professional secrecy.
