The matter is referred to the Commission by registered letter with acknowledgement of receipt. The request for conciliation must state the name and address of the applicant, the name and address of the professional, establishment, health centre or service, the producer, operator or distributor of health products or the promoter of biomedical research involved, as well as the subject of the dispute.

La demande de conciliation mentionne les nom et adresse du demandeur, ceux du professionnel, de l’établissement, du centre ou du service de santé du producteur, de l’exploitant ou du distributeur de produits de santé ou du promoteur de recherche biomédicale mis en cause, ainsi que l’objet du litige.
