Article R141-5 of the French Code governing the entry and residence of foreign nationals and the right of asylum

The application for registration shall be accompanied by all relevant details, and in particular the following information:
1° Language or dialect for which registration is requested;
2° Titles or diplomas of the applicant, in particular in his speciality, literary, scientific or professional works he has accomplished, functions he has performed, activities he has exercised;
3° Professional activities at the date of the application;
4° Qualification of the applicant in his speciality;
5° Means of telecommunication and facilities available to the applicant.

La demande d’inscription est assortie de toutes précisions utiles, et notamment des renseignements suivants :
1° Langue ou dialecte pour lesquels l’inscription est demandée ;
2° Titres ou diplômes du demandeur, notamment dans sa spécialité, travaux littéraires, scientifiques ou professionnels qu’il a accomplis, fonctions qu’il a remplies, activités qu’il a exercées ;
3° Activités professionnelles à la date de la demande ;
4° Qualification du demandeur dans sa spécialité ;
5° Moyens de télécommunication et installations dont le candidat peut disposer.
