Article R2141-15 of the French Public Health Code

The application for authorisation must be sent to the Director General of the Agence de la biomédecine by any means capable of establishing a date of receipt.

This application shall include a file containing all the information required to verify compliance with Articles 16 to 16-8 of the Civil Code and the provisions of this Title, the model for which shall be laid down by decision of the Director General of the Agence de la biomédecine.

La demande d’autorisation est adressée au directeur général de l’Agence de la biomédecine par tout moyen permettant de donner date certaine à sa réception.

Cette demande comprend un dossier qui comporte tous les éléments nécessaires à la vérification du respect des articles 16 à 16-8 du code civil et des dispositions du présent titre, dont le modèle est fixé par décision du directeur général de l’Agence de la biomédecine.
