I. – The application for authorisation must be accompanied by a file containing :
1° The name and address of the applicant ;
2° Where applicable, the articles of association and composition of the board of directors of the applicant organisation;
3° A description of the applicant’s main activities;
4° Where applicable, the organisation chart of the organisation carrying out the health inspections of ships.
5° The full name, position, professional qualifications and diplomas of the person responsible for ship health inspections;
6° The port for which approval is sought;
7° The organisation set up by the applicant to provide the service, specifying the equipment and materials made available, the organisational arrangements for dealing with any inspection requests and, where the applicant is an organisation, the number of employees per site who can carry out health inspections;
8° Any experience acquired in the field of health inspection or ship inspection;
9° A sworn statement certifying its commitment to confidentiality, impartiality and independence with regard to ship health inspection activities.
II. – The application is deemed to be complete if the Prefect has issued an acknowledgement of receipt or has not informed the applicant, within one month of receipt, by letter with acknowledgement of receipt, of the list of missing or incomplete documents.
A person or organisation legally established in another Member State of the European Union or party to the Agreement on the European Economic Area shall submit the documents required by this article in support of its application for authorisation. If this person or organisation has obtained a qualification of equivalent effect in its country of origin or has technical capabilities equivalent to those mentioned in article R. 3115-39, it must produce the necessary supporting documents.
