Article R4412-57 of the French Labour Code

If the establishment or occupational health service to which the worker is attached disappears or if the worker moves to another establishment, the entire medical file is sent to the occupational health inspector, who is responsible for sending it, at the worker’s request, to the occupational health physician now responsible.

Si l’établissement ou le service de santé au travail auquel il est rattaché vient à disparaître ou si le travailleur change d’établissement, l’ensemble du dossier médical est transmis au médecin inspecteur du travail, à charge pour celui-ci de l’adresser, à la demande du travailleur, au médecin du travail désormais compétent.
