If the worker has had an information and prevention visit within the last five years or, in the case of the worker referred to in Article R. 4624-17, within the last three years prior to his recruitment, a new information and prevention visit is not required if all of the following conditions are met:
1° The worker is to occupy an identical job with equivalent exposure risks;
2° The health professional referred to in the first paragraph of Article L. 4624-1 is in possession of the most recent follow-up certificate or the most recent fitness report;
3° No measure formulated under article L. 4624-3 or no opinion of unfitness issued in application of L. 4624-4 has been issued in the last five years or, for the worker mentioned in article R. 4624-17, in the last three years.
