Any production or dispensing by a pharmacist of a magistral or officinal preparation is immediately recorded in a register book or by any appropriate system.
Each transcription or record includes a different chronological serial number and the following information:
-the date on which the preparation was made or dispensed ;
-the name and address of the prescriber for magistral preparations;
-the name and address of the patient, when the dispensing is transcribed or recorded, and, in the case of a veterinary magistral preparation, the surname, first name and address of the keeper of the animals, the identification of the animals with regard to their species, age, sex, identification number or any means of identifying the batch of animals;
-the complete qualitative and quantitative composition of the preparation, indicating the batch number of each raw material and the name of the supplier;
-the quantity produced or dispensed, with an indication of the mass, volume and number of dosage units in the case of unit forms;
-identification of the person who made the preparation.
When registering or recording the dispensing of a pharmacopoeial preparation, its composition is replaced by the production order number.
The registration systems allow, at the request of any control authority, immediate printing of the above data. Each page printed includes the name and address of the pharmacy. The data contained in these systems must not be modified after validation of their registration. They must be stored on a medium that guarantees their permanence and integrity. They must be duplicated and stored on two separate media, the first used for normal consultation, the second kept in reserve. Archived data must be accessible, consultable and usable for a period of ten years.
