The internal-use pharmacy may carry out, on its own behalf or within the framework of cooperation on behalf of other internal-use pharmacies, all or some of the tasks provided for in 2° and 3° of Article L. 5126-1.
Clinical pharmacy activities are as follows:
1° Clinical pharmaceutical appraisal of prescriptions involving medicinal products, products or objects mentioned in article L. 4211-1 and sterile medical devices, for the purpose of ensuring the therapeutic follow-up of patients;
2° Carrying out medication reviews as defined in article R. 5125-33-5;
3° Drawing up personalised pharmaceutical plans in collaboration with the other members of the healthcare team, the patient and, where appropriate, their family and friends;
4° Pharmaceutical interviews and other therapeutic education initiatives for patients;
5° Drawing up a therapeutic strategy to ensure the relevance and efficiency of prescriptions and to improve the administration of medicines.
The actions mentioned in 2°, 3°, 4° and 5° may be carried out as part of the care team mentioned in article L. 1110-12.
