Article R5141-144 of the French Public Health Code

The file requesting the opinion shall include the following information:

1° For agreements concerning the research and scientific evaluation activities referred to in the third paragraph of Article L. 5141-13-1 :

a) The unsigned agreement, indicating the name, company name and registered office address of the company proposing it;

b) The amount and method of calculating the remuneration of veterinary surgeons or the allowance of veterinary students and, where applicable, the nature of any other benefits they may receive;

c) A list of the names of the veterinary surgeons, indicating their registration number with the Order of Veterinary Surgeons, or, in the case of veterinary students, a list of the names of the students, indicating the year of their course and the educational establishment to which they belong;

d) A summary of the research or scientific evaluation protocol;

e) A draft observation book, in compliance with the rules of good laboratory practice or good clinical practice mentioned in article L. 5141-16 for experiments involving veterinary medicinal products or the data collection document provided for in the protocol for other research or scientific evaluation activities;

2° For agreements relating to hospitality offered, directly or indirectly, during promotional events or events of an exclusively professional and scientific nature, as referred to in the fourth paragraph of Article L. 5141-13-1:

a) The unsigned agreement, indicating the name, company name and registered office address of the company offering the hospitality;

b) The programme for the event;

c) A list of the names of the veterinary surgeons who have been approached, indicating their registration number with the Order of Veterinary Surgeons, or, in the case of veterinary students who have been approached, a list of the names of the students, indicating the year of their course and the educational establishment to which they belong;

d) The nature and amount of each of the services or, where applicable, the package listing the various services paid for during the event in question.

Article R5141-144

Le dossier de demande d’avis comporte les renseignements suivants :

1° Pour les conventions qui ont pour objet les activités de recherche et d’évaluation scientifique mentionnées au troisième alinéa de l’article L. 5141-13-1 :

a) La convention non signée, indiquant le nom, la raison sociale et l’adresse du siège social de l’entreprise qui la propose ;

b) Le montant et les modalités de calcul de la rémunération des vétérinaires ou de l’indemnité des étudiants vétérinaires et, le cas échéant, la nature de tous les autres avantages susceptibles de leur être procurés ;

c) La liste nominative des vétérinaires, indiquant leur numéro d’inscription à l’ordre des vétérinaires, ou, pour les étudiants vétérinaires, la liste nominative des étudiants, indiquant l’année de leur cursus et l’établissement d’enseignement dont ils relèvent ;

d) Le résumé du protocole de recherche ou d’évaluation scientifique ;

e) Le projet de cahier d’observations, conforme aux règles de bonnes pratiques de laboratoire ou de bonnes pratiques cliniques mentionnées à l’article L. 5141-16 pour les expérimentations portant sur les médicaments vétérinaires ou le document de recueil des données prévu par le protocole pour les autres activités de recherche ou d’évaluation scientifique ;

2° Pour les conventions qui ont pour objet l’hospitalité offerte, de manière directe ou indirecte, lors de manifestations de promotion ou lors de manifestations à caractère exclusivement professionnel et scientifique, mentionnée au quatrième alinéa de l’article L. 5141-13-1 :

a) La convention non signée, indiquant le nom, la raison sociale et l’adresse du siège social de l’entreprise qui la propose ;

b) Le programme de la manifestation ;

c) La liste nominative des vétérinaires sollicités, indiquant leur numéro d’inscription à l’ordre des vétérinaires, ou, pour les étudiants vétérinaires sollicités, la liste nominative des étudiants, indiquant l’année de leur cursus et l’établissement d’enseignement dont ils relèvent ;

d) La nature et le montant de chacune des prestations ou, le cas échéant, du forfait énumérant les différentes prestations prises en charge à l’occasion de la manifestation considérée.
