Article 148-6 of the French Code of Criminal Procedure

Any application for the release or modification of judicial supervision or for release must be declared to the registrar of the investigating court hearing the case or to the registrar of the court with jurisdiction under article 148-1.

It must be recorded and dated by the registrar, who signs it together with the applicant or his lawyer. If the claimant is unable to sign, this is noted by the court clerk.

Where the person or his lawyer does not reside within the jurisdiction of the competent court, the declaration to the court clerk may be made by registered letter with acknowledgement of receipt.

Article 148-6

Toute demande de mainlevée ou de modification du contrôle judiciaire ou de mise en liberté doit faire l’objet d’une déclaration au greffier de la juridiction d’instruction saisie du dossier ou à celui de la juridiction compétente en vertu de l’article 148-1.

Elle doit être constatée et datée par le greffier qui la signe ainsi que le demandeur ou son avocat. Si le demandeur ne peut signer, il en est fait mention par le greffier.

Lorsque la personne ou son avocat ne réside pas dans le ressort de la juridiction compétente, la déclaration au greffier peut être faite au moyen d’une lettre recommandée avec demande d’avis de réception.
