Article A212-166 of the French Sports Code

The candidate referred to in article A. 212-165 wishing to obtain a certificate of qualification and aptitude must compile a file comprising :
1° An application on plain paper;
2° An individual civil status form less than three months old;
3° A medical certificate stating that there are no contraindications to practising and teaching the sport in question;
4° An extract from the criminal record;
5° Any documents proving the applicant’s level of knowledge, experience and professional skills;
6° Any documents proving the applicant’s qualifications.
This application must be submitted to the departmental directorate for youth, sport and community life in the candidate’s place of residence. The Regional Director for Youth, Sport and the Voluntary Sector will issue an opinion on the application, which will then be forwarded to the Director of Sport for submission to the qualified jury mentioned in article A. 212-164.

Le candidat mentionné à l’article A. 212-165 désirant obtenir l’attestation de qualification et d’aptitude constitue un dossier comprenant :
1° Une demande sur papier libre ;
2° Une fiche individuelle d’état civil datant de moins de trois mois ;
3° Un certificat médical de non-contre-indication à la pratique et à l’enseignement du sport concerné ;
4° Un extrait du casier judiciaire ;
5° Toutes pièces permettant de justifier le niveau de connaissances, l’expérience et les capacités professionnelles du candidat ;
6° Toutes pièces permettant d’apprécier les titres dont le candidat prétend se prévaloir.
Ce dossier sera déposé à la direction départementale de la jeunesse, des sports et de la vie associative du lieu de domicile du candidat. Il fait l’objet d’un avis du directeur régional de la jeunesse, des sports et de la vie associative, puis est transmis au directeur des sports afin d’être soumis au jury qualifié mentionné à l’article A. 212-164.
