The official statement of offence referred to in Article A. 37-1 is orange in colour and its characteristics are as follows:
I.-On the front, on the left-hand side, are the details provided for in I of article A. 37-9, which are established by duplicating the left-hand side of the second section.
II.-The official statement of offence includes the words: “This offence results in the deduction of point(s) from the driving licence. “
III.-On the right-hand side are spaces for the signature of the ticketing officer and, where applicable, for the figures used to process the ticket recorded, for the establishment of an immobilisation sheet, for the signature and statements of the offender indicating whether or not he acknowledges the offence and specifying that he acknowledges having received the payment card and the notice of ticket on which the information provided for in III of Article A. 37-9 and reproduced on the back of the form appears.
IV.-On the back, on the left-hand side, all the information provided for in III of Article A. 37-9 is reproduced and it is indicated that the offender has received the notice of offence containing this information.
V.-On the right-hand side is the information relating to the offender and, where applicable, the holder of the registration certificate.
