The front and back of sheet no. 1 of the bundle include a reminder of the legislative and regulatory texts applicable to contraventions subject to the fixed fine procedure.
The fronts of slips nos 2, 3, 4 and 5, which contain identical information by duplication effect, are intended to collect the information needed to identify the issuing service and the offender or perpetrator, the offence or offences recorded and the date and place of collection.
A section entitled “Collection” is intended to record the amount of the fixed fine that has been paid or the amount of the deposit that has been paid as well as the method of payment.
The back of slip no. 2 includes the information relating to the rights of the offender provided for in 1, 2, 3 (first paragraph), 4, 5 and 6 of III of article A. 37-9.
Slips nos 2, 3 and 5 are signed by the ticketing officer.
Slips nos 1 and 2 are given to the offender.
Sheet nos 3 is given to the public accountant when the funds are paid by the ticketing officer.
Sheet No. 5 remains in the collection counterfoil book.
