Article A37-5 of the French Code of Criminal Procedure

The characteristics of the official statement of offence referred to in Article A. 37-1 are as follows:

I.-On the front, on the left-hand side, the information provided for in I of article A. 37-4, which are established by duplicating the left-hand side of the second section.

II.-On the right-hand side are spaces for the signature of the ticketing officer and, where applicable, for the figures used to process the fine recorded, for the establishment of an immobilisation sheet, for the signature and statements of the offender indicating whether or not he acknowledges the offence and specifying that he acknowledges having received the payment card and the notice of fine on which the information provided for in II of Article A. 37-4 and reproduced on the back of the form appears.

III.-On the back, on the left-hand side, all the information provided for in II of Article A. 37-4 is reproduced and it is indicated that the offender has received the notice of offence containing this information.

IV.-On the right-hand side are details of the offender and, where applicable, the holder of the vehicle registration certificate.

Article A37-5

Les caractéristiques du procès-verbal de contravention mentionné à l’article A. 37-1 sont les suivantes :

I.-Au recto, sur la partie gauche, sont portées les mentions prévues par le I de l’article A. 37-4, qui sont établies par duplication de la partie gauche du deuxième volet.

II.-Sur la partie droite figurent les emplacements destinés à la signature de l’agent verbalisateur et, le cas échéant, aux éléments chiffrés permettant le traitement de la contravention relevée, à l’établissement d’une fiche d’immobilisation, à la signature et aux déclarations du contrevenant indiquant s’il reconnaît ou ne reconnaît pas l’infraction et précisant qu’il reconnaît avoir reçu la carte de paiement et l’avis de contravention sur lequel figurent les informations prévues au II de l’article A. 37-4 et reproduites au verso du formulaire.

III.-Au verso, sur la partie gauche, est reproduite l’intégralité des informations prévues au II de l’article A. 37-4 et il est indiqué que le contrevenant a reçu l’avis de contravention comportant ces informations.

IV.-Sur la partie droite figurent les informations relatives au contrevenant ainsi que, le cas échéant, au titulaire du certificat d’immatriculation.
