CONTENT OF THE MANAGEMENT SHEET FOR SECOND-HAND PERSONAL PROTECTIVE EQUIPMENT
SUBJECT TO THE PROVISIONS OF THE SPORTS CODE

The management sheet referred to in article A. 322-177 contains the following information:

– identification and characteristics of the equipment: the precise reference of the equipment, the manufacturer’s instruction manual (or a copy thereof), the date of purchase or, failing that, of commissioning, the planned date of disposal for personal protective equipment subject to ageing;

– maintenance of conformity: a description of the organisation put in place to ensure that the equipment is maintained in conformity with the instructions in the manufacturer’s manual, the nature of any repairs carried out, the nature and date of any incidents involving the equipment, a dated indication of the replacement of interchangeable components;

– hygiene and disinfection measures: nature and monitoring of measures according to the frequency of hire or availability;

– the effective date of scrapping or removal of the equipment from stock.

INDIVIDUELLE D’OCCASION SOUMIS AUX DISPOSITIONS DU CODE DU SPORT

La fiche de gestion visée à l’article A. 322-177 comporte les informations suivantes :

– identification et caractéristiques de l’équipement : la référence précise de l’équipement, la notice d’instructions du fabricant (ou une copie de celle-ci), la date d’achat ou, à défaut, de mise en service, la date prévue de mise au rebut pour les équipements de protection individuelle sujets à vieillissement ;

– maintien en état de conformité : la description de l’organisation mise en place pour assurer le maintien en état de conformité de l’équipement en fonction des instructions figurant sur la notice du fabricant, la nature des réparations réalisées, la nature et la date des incidents survenus sur l’équipement, l’indication datée du remplacement d’éléments interchangeables ;

– mesures d’hygiène et de désinfection : nature et suivi des mesures en fonction du rythme des locations ou des mises à disposition ;

– la date effective de mise au rebut ou de sortie du matériel du stock.
