The employee identification form provided for in 2° of article D. 1272-1 contains the following information:
1° Information relating to the employee:
a) All the information provided for in article R. 1221-1, relating to the declaration prior to recruitment;
b) The employee’s affiliation to the general scheme or the agricultural scheme;
2° Information relating to the employment:
a) The end date of the job if it is a fixed-term job;
b) The length of the trial period;
c) The expected salary on recruitment;
d) The working hours;
e) The nature and category of the job;
f) The applicable collective agreement;
g) The rate of industrial accident contributions and, where applicable, the provident fund rate;
3° The signatures of the employer and the employee.
