Article D3111-6 of the French Public Health Code

The declaration provided for in article L. 3111-5 is made:

1° On the health record and, in addition, for children aged two years or less, on the health certificates provided for in article L. 2132-2 ;

2° For persons who do not have a health record, on a document issued by a health professional authorised to carry out vaccinations certifying the person’s status with regard to compulsory vaccinations.

The health certificates referred to in 1° are sent to the doctor in charge of the departmental maternal and child protection service under the conditions set out in article L. 2132-3.

Article D3111-6

La déclaration prévue à l’article L. 3111-5 est faite :

1° Sur le carnet de santé et, en outre, pour les enfants âgés de deux ans ou moins, sur les certificats de santé prévus à l’article L. 2132-2 ;

2° Pour les personnes ne possédant pas de carnet de santé, sur un document remis par un professionnel de santé autorisé à pratiquer les vaccinations attestant de la situation de la personne au regard des vaccinations obligatoires.

Les certificats de santé mentionnés au 1° sont adressés au médecin responsable du service départemental de protection maternelle et infantile dans les conditions fixées par l’article L. 2132-3.
