Article D4625-29 of the French Labour Code

The main prevention and occupational health service is informed by the employer within one month of joining the local prevention and occupational health service:

1° Details of the local prevention and occupational health service ;

2° The names and contact details of the occupational physicians and health professionals referred to in the first paragraph of Article L. 4624-1 who are competent ;

3° The list of workers monitored by the local prevention and occupational health service, including those subject to reinforced individual monitoring.

Le service de prévention et de santé au travail principal est informé, par l’employeur, dans le délai d’un mois après son adhésion au service de prévention et de santé au travail de proximité :

1° Des coordonnées du service de prévention et de santé au travail de proximité ;

2° Du nom et des coordonnées des médecins du travail et des professionnels de santé mentionnés au premier alinéa de l’article L. 4624-1 compétents ;

3° De la liste des travailleurs suivis par le service de prévention et de santé au travail de proximité, dont ceux relevant d’un suivi individuel renforcé.
